By completing this Equipment Repair form, I understand I will be responsible for the non-refundable fee associated with the Cleaning and Maintenance Package.

I understand Sjolie Sunless is not responsible for any additional fees required to repair my equipment fully beyond the package price, and I will be responsible for said fees.
I understand I am responsible for all shipping fees of equipment to and from the Sjolie Facility.
I understand the maintenance of my equipment could take up to 30-days based on the severity of the equipment.
I understand that if maintenance is beyond the service and repair provided by Sjolie Sunless, a representative will contact me with the recommended action, resulting in the purchase of new equipment and replacement parts. The original fees paid are considered non-refundable.
I understand Sjolie Sunless will do their best to avoid damage to equipment; however, cosmetic scratches or dents may result from the necessary maintenance procedure. In no way is Sjolie Sunless obligated to reimburse for costs of damage.
